When you are offered a job with Qantas or at one of its subsidiary companies, you will be sent a letter of appointment and new starter kit which includes the following documentation:

• Product Disclosure Statement
  (You need to check-in to access a copy of the PDS that relates to you)
• Voluntary Contribution Advice Form - Regular Contributions
• Nomination of Beneficiary Form
• Compliance Letter (for roll-overs)
• Application Form (only for regional airlines employees)

After you start working at Qantas or at one of its subsidiary companies, you will be sent a letter from the Plan to confirm your membership and welcome you to the Plan. Along with the letter, you will also be sent the following:

• Latest copy of the Plan’s Annual Report; and
• Nomination of Beneficiary Form

After becoming a member, you may establish a Spouse Member Account for an eligible spouse. An application form is available in the back of the Product Disclosure Statement.
